Thoughts on Whiplash or Neck Trauma by a NYC Chiropractor/Applied Kinesiologist

“Whiplash” is commonly associated with auto accidents but also can be associated with many types of trauma like sport injuries, bad falls and even nodding off in chair and awaking with a sudden jerking of the neck.

The term “whiplash” refers to the mechanism of the injury. The proper terminology is cervical hyperextension/hyperflexion sprain. Hyperextension means that the head and neck are bent backward beyond their normal range of movement. Hyperflexion means that the head and neck are bent forward beyond the normal range of motion. Symptoms can include neck and back pain, loss of range of motion, shoulder & arm pain or numbness, jaw pain, dizziness, headaches, nausea, difficulty swallowing etc.

Within the neck region’s joints are some of the most complex movements within the body. The cervical bones house and protect the spinal cord. Nerves from the cord pass out of the spine between the vertebrae; nerve entrapment can happen due to the shift of the cervical joint and spasm in both the smaller vertebral muscles http://www.dummies.com/how-to/content/the-prevertebral-muscles-of-the-neck.html that control fine movements and the larger and longer muscles https://www.realbodywork.com/learn/neck/neck.htm that control the gross movements of the neck which can result in the above

cervical muscles

Cranial-Sacral Mechanism: the skull and sacrum operate in ways that are often inadequately considered in whiplash cases. There is movement between the cranial bones and the sacrum called the cranial sacral respiratory mechanism that pumps cerebrospinal fluid https://drvittoriarepetto.wordpress.com/2010/05/17/cranial-sacral-therapy-in-applied-kinesiology/ up and down the spine that helps keep the nerves healthy. Whiplash can cause jamming of the cranial bones resulting in poor function. Poor function of this mechanism can interfere with cranial nerves, some of which control neck muscles, muscles that move the eyes, and muscles that move the jaw, muscles involved with swallowing and breathing.

Some of the very bizarre symptoms of whiplash trauma develop as a result of cranial dysfunction. Intricately associated with this area are the nerves responsible for balance. These include cranial nerve VIII, which supplies the balance mechanism of the middle ear, cranial nerves III, IV, and VI that supply the muscles that move the eyes and are intricately associated with the visual righting reflexes, and the nerve endings in the upper cervical vertebral ligaments that supply the head on-neck reflexes. These reflexes must work together. If there has been injury causing improper nerve supply to one or more of these areas, neurologic disorganization develops that can cause a change in muscle function throughout the body; there may be dizziness, ear ringing, nausea, blurred or double vision, headaches, and myriad other symptoms.

Many whiplash patients develop pain in the jaw joint, called the temporomandibular joint (TMJ), which is aggravated by chewing. https://drvittoriarepetto.wordpress.com/2013/06/26/tmj-problems-jaw-problems-and-how-a-nyc-chiropractorapplied-kinesiologist-handles-the-problem/

As a doctor of chiropractor who also does applied kinesiology, I not only deal with restoring the proper movement of the cervical vertebrae https://drvittoriarepetto.wordpress.com/2010/07/17/the-subluxationspinal-joint-dysfunction/ but almost dealing with the injury to the both the flexion and extension muscles of the cervical area mentioned above whether they need to be relaxed or whether they need to be strengthened. https://drvittoriarepetto.wordpress.com/2010/06/21/muscle-balancing-in-applied-kinesiology/

Nutrition is suggested to help repair injured muscle fibers.

Cranial bone movement is checked via muscle testing and restored by non-force movements with inspiratory assistance

The movement of the sacrum, commonly missed in a non-applied kinesiology office, is checked for dysfunction and adjusted for return of normal function and proper flow of the cerebrospinal fluid so important to the health of the brain and the spinal cord and nerves.

The Use of Applied Kinesiology in a Chiropractic Examination

In the chiropractic examination which includes orthopedic, neurological and spinal palpation, the addition of applied kinesiology and muscle testing adds an extra dimension of revealing the patient’s health status.

In the 1960’s, Dr George Goodheart found that muscle testing could be used in the evaluation of normal and abnormal body function. His examination soon included evaluation of the vascular and lymphatic system, nutrition, acupoint therapy, cranial movement and other factors that control health. He called this system based on the application of muscle testing: applied kinesiology.

Muscle testing is the first part of the applied kinesiological examination; muscles are tested in a specific manner in different areas of the body. When a weakness is found, the question that the chiropractic/AK doctor has to answer is why the weakness is there.

Is the weakness due to a spinal/nerve problem, a vascular problem, a problem with lymphatic function, a nutritional default, a problem with organ function or an acupoint associated w/ that muscle?

AK-img3

What treatment is needed to correct the problem and improve the patient’s health?

In the process of answering the question, the patient will either place their finger or hand on specific points or areas and the doctor retests the muscle to see if the weakness is corrected. This is called therapy localization.

If the therapy localization is positive and the area involved is the spine or a joint or a cranial fault,, the doctor will move the area in a way to stimulate neuro/mechano receptors in the joint or spine. This is called a “challenge’ and shows the direction of manipulation needed to improve function of the joint/spine and/or cranial movement.

Therapy localization is also used to evaluate if there is a problem w/ the vascular and lymphatic that supply the organs of the body or the meridian/acupoint system that are related to certain organ and muscle function.

In addition to the information from blood tests and examination of hair, eyes, skin, etc, an applied kinesiologist can use muscle testinng to “challenge” the supplements needed or the possibility of food allergies or intolerances that are affecting the patient’s health.

Therefore the addition of applied kinesiology combined with usual examination helps find what the problem is and how to correct it. The correction immediately improves muscle function and decreases the pain or the problem that the patient first came in with to the doctor.

On future visits, the above examination reveals if the corrections held and if the patient’s health is improving.

When health is restored , an applied kinesiologist uses these methods to maintain health and correct problems before they develop.

Applied Kinesiology As Part Of Treating for Functional Wellness

First what is functional wellness?

Functional wellness is an approach to the human body that allows you to treat the underlying cause of the problem such as for example heartburn—not just manage the symptoms of it.

Its primary distinction is a focus on overall health—physical, mental, and nutritional. It’s not a single treatment for a single problem (or its symptoms).

As a certified applied kinesiologist, I look for the cause of the problem. For example, in a case of heartburn, I look to see if the spinal segments innervating the stomach, the esophagus and diaphragm are moving freely.

Do the neuro-lymphatic or the neuro-vascular points need stimulation to increase lymphatic or vascular flow?

Is there a cranial fault involved?

Is the patient using their diaphragm correctly to breathe?  Is an imbalance in their psoas muscle affecting their diaphragm?

Is a visceral manipulation needed?

Does the patient need enzymes to help them digest or zinc to heal inflammation in the digestive tract?  Does the patient need probiotics?

Are there acupressure points involved? Is the energy in their stomach meridian being affected by an overflow of liver meridian energy due to the patient’s anger issues?

These and other questions arise when looking for the cause of a problem.

The human body functions as a network of interconnected systems that affect each other. so a functional wellness approach such as a chiropractic/applied kinesiology/nutrition treats the whole body rather than a sub-section or single organ system.

The goal of functional wellness also is the prevention and addressing the underlying clinical imbalances that may lead to any number of chronic problems.

Keeping the spine and the cranium moving freely and the lymphatic, vascular and acu-points unblocked and making sure that the patient has the proper nutrition that they need is part of our prevention/maintenance care.

What is Applied Kinesiology?

Applied Kinesiology (AK) can be defined as the clinical application of the study of movement and function; it is functional neurology.

AK is a system of testing and treatment that is used to assess the body’s functional aberrations.

These functional aberrations may be but not limited to:

    •   Muscular
    • Neurological
    • Vascular
    • Osseous
    • Lymphatic
    • Respiratory
    • Digestive
    • Endocrine
    • Imbalance in the Chinese Meridian systems
  •

Therapies to induce or restore individual normal function include but not limited to:

    • Chiropractic adjustive therapy
    • Cranial techniques
    • Therapeutic massage modalities
    • Reflex therapies
    • Acupuncture therapies
    • Exercise & stretches
    • Nutritional supplementation
    • Emotional support or modalities
  • Lifestyle changes

With the aim of decreasing cumulative noxious stimuli below threshold and allowing the body to heal itself.

AK does not replace standard examination procedures such as neurological and orthopedic testing, blood laboratory work or X-rays but rather it is another examination tool at the doctor’s disposal.

Applied Kinesiology (AK) is a continually evolving system that provides the doctor with the skills and knowledge to purposefully, systematically and logically ascertain the optimal treatment of the patient. AK embraces the work of all individuals and disciplines who endeavor to diminish the suffering of humanity.

AK attempts to unify such diverse knowledge and techniques into a usable scientific system for one purpose: To bring patients to a higher level of health and help them to actualize their potential.(Thanks to Dr. Charles for the use of his definition.)

Applied Kinesiology Is Not:

Testing nutritional needs by holding bottles in your hand or placing pills on the skin.

Touch for health or any other forms of evaluation using muscle testing as a simple yes-no answer system.

Testing using mental telepathy.

A simplistic cookie cutter approach to treatment.

A NYC Chiropractor’s/ Applied Kinesiologist’s Office Statement to Her New Patients

Welcome to My Office!

You have entrusted me with your health and wellness. I take that responsibility seriously by continuing my study of the latest chiropractic, applied kinesiological and nutritional research in order to provide the best in care and service to you.

Here you will find a blend of old fashioned doctoring where you are listened to and treated w/ courtesy and dignity, where your time is valuable, where you are not part of today’s assembly line health care and where I use Applied Kinesiology (AK) as part of my practice.

AK, as founded by Dr. George Goodheart, incorporates under its umbrella the many branches of holistic healing including chiropractic, extremity adjustment, muscle balancing, cranial-sacral therapy, TMJ (jaw) tech., nutrition, acu-point therapy, neuro-vascular, neuro-lymphatic therapies and more.

I  also use NeuroKinetic Therapy (NKT) ; NeuroKinetic Therapy works with the concept that movement is performed in systems or patterns. The question of whether one muscle is compensating (facilitating) for another weak or inhibited muscles is a problem looked at by NKT.

My goal is to motivate and help you reach a higher level of health and to actualize your potential. I do this through the ability to determine why the body is not functioning as a system, to get the body working together as it is intended, and help you clear stressors that often impede health and happiness.

I believe that good health is a process and not an event. Therefore I ask that you take responsibility in the healing process as you learn why your health problem has occurred and what you can do to help stop and reverse the problem.

In this process, you might be a little sore after the first or second treatment as I am trying to restore the normal biomechanical motion and proper nerve, muscle and organ function that may have been lost due to years of  bad posture, trauma, infections, faulty nutrition and other stresses.

In other words, I am trying to remove an unhealthy pattern that has developed over the years and this pattern takes awhile to be removed, sometimes a temporary minor relapse may even occur.

Because of this, I will be recommending that you come in for a series of treatments perhaps once a week for a month and than every two weeks until the pattern is broken and health restored. In order to help in this process, I may also be recommending some nutritional supplements, exercises, stretches and lifestyle changes. When I have restored your body to a more normal and healthy function, I will then be recommending a maintenance / preventative program for you.

I’m looking forward to working with you in this process.

How a NYC Applied Kinesiologist/NeuroKinetic Therapist Treats Heartburn or Acid Reflux

Heartburn, also known as acid reflux, is a burning sensation in the chest, just behind the breastbone or just below the breastbone. The sensation of pain or discomfort often rises in the chest and may radiate to the neck, throat, or angle of the jaw. The sensation gets worse on lying down or bending over.

Chronic heartburn is called gastroesophageal reflux disease (GERD). GERD is a relatively new classification of chronic heartburn (less than 10 yrs) which seems to have appeared at the same time as two new classifications of drugs H-2-receptor blockers and Proton pump inhibitors which were stronger and provided longer relief than similar antacids.

However  all three mentioned drug classifications by different mechanisms stop the production of stomach acid; stomach acid which we need in order to not only digest food but help us absorb essential nutrients like Calcium and Vitamin B12.

Now one of the possible causes of heartburn or acid reflux is a hiatal hernia. When one part of the internal body pushes into another part where it should not be, it is called a hernia.  The hiatus is an a opening in the diaphragm muscle that allows for the passage of the esophagus as it attaches to the stomach and the passage of the aorta, the inferior vena cava and nerves.

See http://en.wikipedia.org/wiki/Diaphragm_%28anatomy%29 and observe the way the diaphragm muscle is constructed. There are muscle fibers that wrap around both sides of the esophagus.

A hiatal hernia is when due to diaphragm weakness, the stomach slips upward through the hiatus into the space where the esophagus is causing both nerve irritation and slippage of stomach acid into the esophagus causing the sensation of heartburn or GERD.

hiatal hernia

As a chiropractor, we affect the diaphragm by manipulation of the 4th & 5th cervical vertebrae which  then forms the phrenic nerve and innervates the diaphragm and the stomach and esophagus via manipulation of the greater splanchnic nerve  at 5th to 9th thoracic vertebrae

As an applied kinesiologist,  we work the neuro-lymphatics and neuro-vascular points to increase blood and lymph flow for these organs. I would also look for a possible fixation (not a subluxation) of the lower thoracic vertebrate.

Besides the diaphragm, the psoas muscle is important to check as its fibers blend with the fibers of the posterior diaphragm. The muscles of the chest and the abdomen are also important to check if the patient is constantly in a flexed position.

As an applied kinesiologist, I check to see which muscles are weak or hypertonic.

As a neurokinetic therapist, I look to see if the muscles of the posterior thoracic (mid back)  are inhibited by the muscles of the anterior chest or abdomen .

The cranial system is checked for involvement of the parasympathetic (vagus nerve).

Acupoints  for the stomach, and lung may be checked for involvement.

We would gently after these procedures try to gently manipulate the movement of the stomach back into its proper place and then strengthen the diaphragm muscles; give breathing exercises for the patient to do at home to stop the hernia from happening again are  included as well as nutrients like zinc, l-glutamine to heal any micro-erosions in the GI wall, digestive enzymes to restore the function of the stomach and   supplements like Vitamin B12 and calcium that have not been absorbed due to medications.

Looking for a doctor who will spend time w/ you ?

Or why this NYC chiropractor is not a slave to an insurance company

You hear it all the time: patients complaining about their doctors’ practices: waiting a half hour or more past their scheduled appointment, and where you do see the doctor, you are lucky if you get 10 minutes and get all your concerns voiced, the lack of communication for follow-up via phone or e-mail with the doctor, having to deal w/ leaving messages w/ the answering service.

Part of the problem is that a good number of doctors belong to an insurance network or HMO and due to the fact that they are paid very little per patient and they still have to hire specialized staff just to handle the paperwork in order to get that small payment, they need to squeeze in as many patients as they can.

Personally I’m not affiliated with any insurance network or as I like to say: I’m not a slave to an insurance company.

You see, I like to find out what is happening with my patient.

I like to be able to take time to, for example, balance the muscle pull on their spine or extremities or to do cranial sacral work or acupressure work or talk to them about their nutritional needs or suggest exercises or lifestyle changes related to their condition.

Another issue is availability to my patients after the visit. Patients frequently have questions and they can reach me either via my cell phone or e-mail.

Because of this personal approach, I charge my patients directly and they pay me upfront.

And then I fill out their insurance claim form and they will get reimbursed by their insurance company if they are allowed to go “out of network” by their policy.

The fact that I do not have to hire additional staff to “run after” my payments allows me to spend quality time with my patients.

Also it allows me to keep my fees reasonable and moderate by New York City standards, I charge $150 for the initial visit which includes consultation, examination and treatment and the $100 for the following visits.

So if you want an old fashion  doctor- patient relationship where the doctor knows who you are and you can trust your doctor, please give me a call at 212-431-3724 and let’s talk.

The Basics of Applied Kinesiology

Applied kinesiologists use the following skills:

As some of you know, Muscle Testing is a basic in AK. When you test a muscle, you test not only the condition of the muscle itself, but the joint it crosses or moves, the spinal innervations of that muscle, the Chinese meridian (acupuncture energy line) associated w/ that muscle and the organ associated w/ that Chinese meridian and some cranial-sacral faults.

The Art and Science of Muscle Testing in Applied Kinesiology

Challenge is an essential diagnostic procedure used to determine the body’s ability to cope with external stimuli, which can be physical, chemical, or mental.  An example of a physical challenge is pushing on a skeletal joint to determine an irritation that produces a muscle strength change. Chemical challenge may occur when one inhales potentially toxic chemicals or chews nutritional factors.  Mental challenge includes thought processes, either pleasant or unpleasant to the individual. After an external stimulus is applied, muscle-testing procedures are done to determine a change in the muscle strength as a result of the stimulus.

Therapy localization is another diagnostic procedure in AK that consists of placing the patient’s hand over areas of suspected involvement, then using muscle testing procedures to determine any change in strength. Placing the patient’s hand on different locations stimulates nerve endings and/or possibly changes the patient’s electromagnetic energy field. Therapy localization is strictly a diagnostic tool in AK that is to be combined with the other diagnostic findings to arrive at a final conclusion.

Nutritional evaluation in AK is done as part of your total examination. The muscle test is used to confirm the other findings your doctor will use from laboratory testing, nutritional diaries, blood and saliva testing, and your history. Evaluation of nutritional products and foods by using taste to determining how your body reacts to them, as observed by MMT(manual muscle testing) , is an important addition to providing optimal nutritional support for patients who need it.

For more information on  nutritional evaluation, please read: https://drvittoriarepetto.wordpress.com/2010/01/17/the-correct-way-to-do-nutritional-evaluation-by-muscle-testing-in-applied-kinesiology

Cranial Sacral Therapy in Applied Kinesiology

Cranial Sacral Therapy is a technique taught in Applied Kinesiology based on Cranial Osteopathy which was discovered by osteopath Dr. William Sutherland after he had a remarkable insight while examining the specialized articulations of cranial bones. Contrary to popular belief Dr Sutherland realized that cranial sutures were, in fact, designed to express small degrees of motion.

cranial sutures

DeJarnette and Goodheart introduced diagnostic methods for the evaluation and treatment of cranial dysfunctions. The key technical factor that has advanced cranial diagnosis and treatment, and brought the entire field of cranial therapy into accessible, reproducible, practice and scientific form, was provided by Goodheart’s discovery that the musculoskeletal system and manual muscle testing (MMT) reflects what is going on within the cranial mechanism.

MMT has allowed applied kinesiologists to discover the dramatic functional relationships that exist between the cranium and every other articulation and tissue in the body. Furthermore, patients are not treated in a “touchy feely” fashion in which the patient’s skull is cradled for an indeterminate time, until the cradler perceives warmth or a yielding or softening sensation.

There are many other physical signs and tests (besides MMT) that also reveal cranial dysfunction; these have been written about extensively in the applied kinesiology (AK), sacro-occipital technique (SOT) and osteopathic literature. Returning the dura to a physiological range of tension by using specifically applied cranial corrections is a major goal of AK evaluation and treatment, which seeks to achieve zero defects inside and outside the cranium.

Like Cranial Osteopathy, Cranial Sacral Therapy seeks to restore the natural rhythmic movement found between the bones of the skull and the sacrum which is correlated to our inspiration and expiration; in other words, the cranial bones and sacrum move in different directions when we breathe in and in opposite directions when we breathe out.

The purpose of this is to aid the circulation of the cerebrospinal fluid throughout the central nervous system.

And some suggested that CSF flow along the cranial nerves and spinal nerve roots allow it into the lymphatic channels. Restoring normal cranial-sacral rhythm enables the body to function optimally and may alleviate a wide variety of painful and dysfunctional conditions within the body.

Using a soft gentle touch practitioners release restrictions in the cranial-sacral system to improve the functioning of the central nervous system.

There are many results why the motion of the cranial and the sacrum may be disturbed.

For some, it can be caused by the trauma of a difficult birth. Normally since the bones are very flexible, normal movement and the act of crying restores the natural movement.

For others, it could have been caused an accidental bang to the head, a fall, whiplash following an auto accident. Jaw problems can also affect the cranials as when we chew or clench our teeth, there are muscles forces directed to the skull such as the pull of  the   Temporalis muscle on the squamosal suture of the skull. Even the act of holding our breath during physical exertion (we should be breathing out at that moment), can cause a failure of proper cranial and sacral motion.

Dysfunction of the cranial sacral motion can be seen in different problems, even some caused by the entrapment of cranial nerves

as they exit the cranium such as trigeminal neuralgia, headaches, migraines, low back  and disc problems, general weakness on one side of the body, problems w/ visual acuity, low or high blood pressure, a spastic ileo-cecal value, neck flexor weakness, allergies, hypochlorhydria, earaches, loss of balance, tinnitus, dizziness and vertigo, recurring upper cervical (neck) problems and scoliosis

In applied kinesiology, there are techniques to find the dysfunctions (or cranial faults) and to find out how to fix it. The difference in muscle strength when the patient is breathing in or breathing out is one clue. The asymmetrical face is another clue that there may be a problem. Or the doctor can observe what happens to the strength of a muscle when she (or he) presses on certain cranials or sutures; this is called a challenge.

Cranial-sacrum corrections are easily made and if incorporated with the correction of accompanying spinal dysfunction, muscle balancing and proper nutrition, it will have a lasting effect.

– A new patient had a history of not being able to look to her left without turning her body since contacting toxoplasmosis when she was 18; she was now 80 yrs old. She stated that the infection affected her lt eye.  Found that the muscles for lt. lat flexion were weak but therapy localization (touching) of the ethmoid bone that makes up the lateral part of the eye socket made the muscles stronger. So I adjusted the bone with an inspiration assist and the muscles were strengthened and the range of motion was improved; in fact the range was now better than the right side.
