How a NYC Chiropractor /Applied Kinesiologist /NeuroKinetic Therapist Treats Sciatica

First what is sciatica; sciatica is an inflammation of the sciatic nerve, the longest nerve in your body. The sciatic nerve is formed from the Lumbar 4 (L4) to Sacral 3 (S3) segments of the spinal nerves as they exit the vertebrae at the same levels.

It provides motor innervation and sensation to part of the muscles along the lower lumbar vertebrae and the muscles of your hip and pelvis, to the muscles in your thigh esp. the hamstrings the back of your knee and lower leg and the sole of your foot.

When you have sciatica, you can have pain, weakness, numbness, or tingling in any of the fore mentioned areas. It can start in the low back and extend down the back of your thigh to your calf, foot, or even your toes. It’s usually on only one side of your body.

Causes of sciatica include a herniation or degeneration of a disc at the levels mentioned above, subluxation of the vertebrae associated w the sciatic nerve. https://drvittoriarepetto.wordpress.com/2010/07/17/the-subluxationspinal-joint-dysfunction/ or a narrowing of the spinal canal that puts pressure on the sciatic nerve root which is called spinal stenosis.

As a doctor of chiropractic, I would do a proper orthopedic and neurological examination to determine the level of spinal dysfunction/subluxation.

But before adjusting the vertebrae to remove the subluxation, I would test via muscle testing and palpation the muscles innervated by the involved spinal segments or attached to the spinal segments.

A case in point would be the testing of the psoas muscle as part of the muscle attaches to the anterior of the lumbar vertebrae; a weakness or a hyper tonicity of the muscle can cause rotation of the vertebrae and place tension on the sciatic nerve root. http://en.wikipedia.org/wiki/Psoas_major_muscle

The piriformis muscle is another important muscle that needs to be checked as the sciatic nerve passes under the piriformis; a spasm of this muscle can put pressure on the sciatic. http://en.wikipedia.org/wiki/Piriformis_muscle

Other muscles that may be checked are the hamstrings, the lumbar erector, the quadratus lumborum, the gluteus maximus and the gluteus mediu

I ask,  is there a dysfunction in the coordination of muscles working in patterns. Is the above mentioned psoas or piriformis inhibited by the muscles  like the gluteus maximus or the quadratus lumborum that are compensating (facilitating) for weak or inhibited muscles, for example. or vice versa. Muscle imbalance can cause misalignment of the vertebrea they attach to. 

The muscles that are found to be either weak or hyper tonic are balanced by spindle or golgi tendon work and blood flow and lymphathic drainage to the muscle is increased by working on neuro-vascular and neuro-lymphatic points.

I would also test for problems with the illiolumber ligament and the sacrotuberous ligament which help stabilize the lumbar spine and sacrum respectively.

I also examine the pelvis as the pelvis forms the foundation support of the human skeleton, I look to see if the patient is showing what is known in SOT technique as a Category Three pelvic problem.

Category Three occurs when the low back can no longer tolerate the physical stressors placed on it. This can be a sudden one-off event such as a lift, or it can be a pre-existing weakness that is aggravated. Category Three produces pain in the low back and sciatica. Correction involves using blocks under the pelvis in a specific direction and position. What a Mixture of Chiropractic, Applied Kinesiology and Nutrition Can Do

03/28/2014 – Yesterday’s new patient has over 25 yrs of chronic pain complicated  by gluten sensitivity and a resulting leaky gut that has never healed and numerous falls and auto accidents. The list of her diagnoses included frozen shoulder that turned into an acromioclavicular jt separation, two ruptured lumbar discs, TMJ problems, bladder control weakness and peripheral nerve weakness and pain in her feet, gall bladder pain, carpal tunnel, fibromyalgia and chronic fatigue.
 
All her medical doctors could offer her was surgery and pain management; previous surgeries had not resolved any problems and she didn’t want to be “drugged” up; and she wanted wellness care. She was tried of being sick; her body feeling “kinked-up” 
 
I worked on getting mobility into her sacrum, her cranials, her lumbar spine; strengthening her psoas – a major muscle for locomotion and posture, adjusting her feet and working on acu-points for her bladder, gall bladder and stomach and balanced her shoulder muscles and adjusted her a-c joint.
 
I started to tell her things she needed to do at home..like doing Kegel exercises to strengthen muscles around her bladder and not trying to hold urine in ( increases bladder problems) but to go immediately to the bathroom, how to ice her low back and feet, what position to sleep in , what probiotics & prebiotics she needed to repair her leaky gut and decrease the inflammation in her body and positive affirmations to repeat to herself about getting well.
 
Suddenly she looked at me and said that she was noting that sitting was not causing pain or increasing the “kinked-up” sensation…she felt a lot looser …I asked her to move the shoulder w/ the a-c joint problem…movement was pain-free and testing of her lt big toe was testing strong.
 
I’m sure that continued treatments and doing the homework I gave her to do will results in amazing changes in her body and her life.
 
05/10/2014 – Patient came in for her second visit due to financial problems, she can only come in for monthly visits. She has been doing her icing and exercises; though she says that she may have overdone it.
 
She is sleeping better and has less fatique and less overall body pains.
 
Her bladder control is much better with only occasional leaks since she has been doing her Kegel exercises and due to better nerve flow after her last adjustment to her sacrum and lower lumar vertebrae.
 
Her shoulder is moving freely though still with some pain and so her shoulder joint and scapula were adjusted and the spindles of her shoulder muscles were worked on.
 
Recent blood tests were negative for RA and Thyroid problems which were positive before; her Vitamin D is very low.
 
Due to financial problems, she has not gotten the supplements I recommended.
 
Her low back feels better and she was treated again with flexion distraction and pelvic category blocking  for her disc problem and stenosis; her pelvic category has changed to a less severe and painful category.  Her sacrum is moving well. Her big toe movement is better.
 
Her wrist (carpal tunnel) was adjusted again and wrist exercises were recommended.
 
Patient was told to continue w/ Kegel exercises for her bladder, and add toe exercises to her routine. And I asked her to get the supplementals I recommened.

Water and Your Health

Recently one of my regular patients came in for an adjustment; she was on a new job that requires her to be on her feet for eight hours and a good deal of it outdoors.  And she was having some low back pain and numbness in her thigh.

 So after the orthopedic tests, I tested her Psoas muscle, it was weak on the side of her pain. The Psoas is attached to the front of the lumbar spine and a weakness or a spasm of the Psoas unilaterally can rotate the lumbar spine out of alignment and cause problems with the nerve roots as they exit the lumbar spine.

 And as it is almost summer and the weather has been warm I asked her about her intake of water, she had not been paying attention to her consumption of water. In the Chinese system of medicine, the Psoas muscle is associated w/ the Kidney Meridian and represents the element of water. So I asked her to go to my water cooler and have a few drinks of water; when she returned, I tested her again and this time, the Psoas tested strong.

 I’m telling this story to get to a few points.

 One is to remind people to drink lots of water especially during the warm weather: the composition of our bodies is 72% water and that includes our muscles. Dehydration can cause our muscles to lose strength.

 “All chemical reactions in the body take place in water. Every cell in the human body is bathed in water, which contains materials to keep them vibrant. Water is a transporter of nutrients and oxygen for proper function of the body’s tissues. Water helps remove waste from the body. Water acts as a natural air-conditioner through perspiration. Water is essential for digestion & absorption of vitamins and minerals. Water keeps our skin moist & supple, as well as a natural lubricator for our joints and internal organs.” http://www.filtercon.com/water_health.htm

 Therefore dehydration can make us feel fatigued and sluggish; a common complaint compounded by salt loss….I’m talking not only sodium salts but also magnesium and potassium loss.  Try to consume the equivalent of 6-8 -8 oz glasses of water a day; soups and veggies and fruit do supply some water. Coffee, teas, beer, alcohol and (yes) herbal teas all have diuretic effect on our body so they don’t count as they make us lose more water than they add.

 And to drink water at regular intervals; if you wait until you feel thirsty, your body is already dehydrated. This rule of thumb becomes more important as we age because as we get older, that part of our brain does not signal as efficiently as it once did.

 Another point is that our spinal disc have less blood vessels perfusing them than the rest of our bodies which means that we get less water to our spinal discs So if you’re thirsty, your spinal discs are drying up and more subject to degeneration; a common cause of back and neck pain. http://www.spine-health.com/conditions/degenerative-disc-disease/degenerative-disc-disease-natural-degenerative-process

Symptoms of overhydration may not be recognized in the early st ages but can include:

  • nausea and vomiting
  • headache
  • changes in mental state (confusion or disorientation)

If left untreated, overhydration can lead to dangerously low levels of sodium in the blood (hyponatremia). This can cause more-severe symptoms, such as:

  • muscle weakness, spasms or cramps
  • seizures
  • unconsciousness
  • coma

To prevent overhydration, individuals should avoid drinking more than one liter per hour of fluid.(1 liter= 4.2 cups; 1 cup = 8 ozs)
